2024-2025 Default Clearance or Overpayment Statement

Contact Information

We have completed the initial review of your 2024-2025 Free Application for Federal Student Aid (FAFSA). Through a database match, the U.S. Department of Education has identified you as being in default on a student loan and/or owing on a federal grant over payment. Please refer to the details below for instructions on resolving your default situation.

You are not eligible to receive any financial aid until your account has been resolved.

  • Please contact the agency or Department of Education at the Service Center indicated on your Student Aid Report (SAR) or contact the National Student Loan Data System (NSLDS) at studentaid.gov for information regarding this matter.
  • When you have resolved the defaulted loan and/or federal grant over payment, attach a copy of your clearance letter along with this form indicating that your account has been resolved. The documentation must state clearly that your default has been cleared and/or that your federal grant over payment has been paid in full and you are again eligible for Title IV federal financial aid.
  • If the information received is incomplete, we will ask you to submit further documentation.

WARNING: If you purposely give false or misleading information on this worksheet, you may be fined, be sentenced to jail, or both.
